The Home Care Medical Social Worker provides services to patients and families geared to enhancing functioning and to addressing psychosocial problems which interfere with recovery from illness or prevent a healthy lifestyle.
The MSW will accept home care referrals from the primary social worker when caseload demand is up, will evaluate patients for community resources, refer appropriate agencies and offer emotional support.
